Transitional care involves managing the continuity of health care during the transition between health care practitioners and facilities as a patient’s condition and care needs change during the course of a chronic or acute illness, which ZephyrLIFE has been designed for.
ZephyrLIFE increases safety, reduces never events and assists clinicians in providing care in hospitals, skilled nursing facilities and homes. Using wearable sensors, wireless communication to the cloud and advanced workflow based algorithms and data display, ZephyrLIFE saves costs, increases revenue.

Zephyr recently completed a 2-year Congestive Heart Failure (CHF) trial of ZephyrLIFE in partnership with Qualcomm Life, Verizon Wireless, NIH, IHS and Flagstaff Hospital in Arizona. The results of the study indicated a 3-fold increase in time between hospital visits and a 2-fold decrease in the cost of patient care.
